J-1 Students: Requesting a New DS-2019

It is important to keep your DS-2019 visa document up-to-date to maintain status. Below is a list of reasons you will need to update your DS-2019.

Change of Major

If you change your major during your academic career at UCI while maintaining J-1 status, it is important to update your document to reflect the changes. You will need to provide the following material with the Student Visa Document Request form:

  • Copy of Unofficial Transcript
  • Copy of I-94 Document

Reinstatement to J-1 Status

You will require an appointment with an J-1 student advisor at the IC for clear reinstatement instructions.

Financial Information Change

If your funding sources change while you are attending UCI, you must request an update of your DS-2019 document. It is important to keep this information up to date. You will need to provide the following materials with the Student Visa Document Request form:

  • Financial Documents
    • Self/Personal Funds: Please provide an original bank statement/letter, in English and in U.S. dollars.
    • Family/Private Funds: Attach the bank statement/letter in English and in U.S. Dollars, or with a conversion in English. In addtion, a signed affidavit of support is required from the sponsor.
    • UCI Funding: You are required to get a signature on the Student Visa Document Request form to verify the amount of funding you will receive from the university.
    • Agency/Goverment Agency/Home University: Provide an original funding letter with the amount of funding you will receive.
  • Copy of I-94 Document

Returning Student

Program Extension

If you are aware that you will not complete your studies, by the end date listed in BOX 5 of the DS-2019 form, you will need to request an extension of your visa document. It is recommended that you submit the request at least 60 days before the end date listed on your DS-2019 to provide the IC time to complete your request. You will be required to provide the following information with the Student Visa Document Request form:

  • Financial Documents: These must be dated within the last 3 months from the date submitted to the IC. The funding must be for at least one academic year, unless your extension is only for one or two quarters, in which case you would only provide 1/3 or 2/3 of the required funds amount.
  • Academic Status Verification: Will need to be completed by an academic advisor.

Change of Status

Replacement of Document

If your document is lost, stolen, or damaged you will need to request a new document. You will need to provide the following with the Student Visa Document Request form:

  • Reason for replacement
  • Copy of I-94 Document

Name Change

The name on your immigration documents must reflect the name on your passport at all times. If you have changed your name, and changed your passport to reflect the change, you will need to update your DS-2019. You will need to provide the following with the Student Visa Document Request form:

  • Passport Biographical Information Page
  • Copy of I-94 Document

Travel

If you no longer have travel signature space [on the 1st page of your DS-2019], you will need to request a new DS-2019. You will need to provide the following items with the Student Visa Document Request form:

  • Proof of Enrollment [print out of class schedule from Student Access or WebReg]
  • Travel Form
  • Copy of I-94 Document
